These are the standard "manual," non-calculating forms.

  • Use the basic child support form, DC 637, for situations where there is joint or sole custody and the children live primarily with one parent and have shorter periods of visitation with the other.
  • The main determining factors in selection of this form are 1) the number of days in the primary home exceed 274, and 2) the children are not split between the parents.

  • The instructions are in Adobe Acrobat format. If your computer does not have the free Acrobat Reader installed, click on the Adobe icon and follow the directions to have it installed now.

  • Use the split custody support form, DC 638, for situations where the parents each have one or more children in their home as the primary residence. For example, if a couple has a daughter and a son and the father takes the son and the mother takes the daughter, use the split custody form.

  • The instructions are in Adobe Acrobat format. If your computer does not have the free Acrobat Reader software installed, click on the Adobe icon and follow the directions to have it installed now.

  • Use the shared custody support form, DC 640, when the parent with the fewer days has 91 days or more with the child and the parent with the most days has 274 days or more. The total number of days needs to add up to 365.
  • Calculate supporat using the basic form too and then select the lowest result. However, note that the law prohibits reducing the child support to a level that endangers the child. See more about this on the law page.
